Appointments

· Please plan on arriving 15 minutes before your appointment
· When coming to your first appointment, please bring a copy of
  any pertinent records, test reports, a complete list of medications
  and previously-used medications, a list of allergies, prior surgeries
  and all treatments you have tried for your problem.
· Generally, the schedule is booked several weeks in advance,
  but follow-up appointments are available for established patients.
· It is important to know that if you cancel an appointment at the
  last minute, you may have to wait several weeks for another
  appointment.
· Failing to keep two consecutive appointments without a good
  reason will result in being discharged from our practice.
· The office does not have childcare facilities and requests that you do
  not bring children to your appointment.
