Billing Information

· Dr. Miller's office participates with a very large number of insurance plans,
  including Medicare, Medicaid, and Molina.
· The offices uses an outside billing service, which will bill your insurance
  company directly for your visit and then send you a bill for the balance
  of what your insurance company does not cover.
· It is your responsibility to know how much of a specialist's office visit
  is covered by your insurance plan.
· It is also critical for you to know how many visits are approved by
  your primary care physician.
